Headache - Definition and classification.

Headache is a very frequently reported complaint by majority of  patients we see in the hospitals / clinics. Any type of  Headache  is not only painful but it also causes significant disability in the sufferer.

According to W.H.O, Headache is among the most common disorder of  nervous system and approximately 47% of the genera population have experienced headache at least once within last year in general. Headache which lasts for more than 15 days per month affects at least 1-4 % of the adult population in the world.

So the question arises, what exactly is a headache? :  According to International Headache society, any pain located above the orbitomeatal line can be called as Headache.

Every patient is unique and may present with variable symptoms. Some of the common presentation frequently encountered in clinical practice are -

1. Patient presenting with an acute single episode of Headache - seen in patients suffering from sinusitis, meningitis, encephalitis, trauma to head.

2. Patients presenting with recurrent acute attack of Headache- seen in patients suffering from migraine, cluster headache and eye disorder like glaucoma.

3. Patients presenting with gradually progressing headache (sub acute Headache)- seen in patients suffering from Giant cell arteritis.

4. Patients presenting with chronic headache (> 15 days per month)- seen in patients suffering from Tension Headache, analgesic abuse and chronically raised intracranial pressure.

According to International Headache society, patient suffering from headache can belong to any one of the 3 groups as mentioned below.

1. Primary Headaches - Including Migraine, Tension Headache, Cluster Headache and other primary headache like post coital headache, cough headache, New daily persistent headache, hemicrania continua etc.

2. Secondary Headache - eg. Headache due to trauma, infection, substance abuse or withdrawal, infection, psychiatric disorder etc.

3. Cranial neuralgia and central cause of facial pain - like trigeminal neuralgia.

According to W.H.O, Migraine, Tension Headache and medication-overuse headache requires special attention.
